The National Health Service (NHS) is, mainly, for the benefit of people living in the United Kingdom (UK). If a person is accepted for NHS treatment it is generally free, although there can be charges for some things. You need to meet the conditions described in this booklet to be exempt from charges for NHS treatment.

Are you claiming free treatment? (factsheet)
Don't assume you're entitled! Certain groups don't have to pay NHS dental treatment charges. You should always check with your dentist whether your treatment is private or NHS. If you're entitled to free NHS treatment and the treatment you receive is a mixture of NHS and private, you will still have to pay for the private treatment.
Claim for for a refund of optical charges - HC5(O)
This form can be used to claim back the cost of a sight test, glasses or contact lenses on low income grounds.
Claim form for a refund of NHS dental charges - HC5(D)
This form can be used to claim back the cost of NHS dental treatment for people on a low income who may be entitled to a refund.
Claim form for a refund of travel costs to receive NHS treatment - HC5(T)
You can claim help with the cost of travel if you are on a low income and have made an additional journey to receive NHS care following a referral by a doctor (GP or hospital doctor) or dentist.
