Healthy living feedback survey (card)
This card should be given to any patient who has had either an NHS Health Check, used a weight management programme, or accessed stop smoking services. It asks them to go to a website to complete an online survey (Healthy Living).

An information pack that can be provided to patients that have received an NHS Health Check. Please note contents may vary slightly to picture due to availability of individual items.
Pharmacy prescription bag stickers (NHS Health Checks)
These stickers have been developed in consultation with pharmacies and are designed to help you promote your NHS Health Check service. If you currently provide NHS Health Checks to your patients we suggest you put these stickers on the prescription bags of people who you think may be eligible for an NHS Health Check i.e. those who are Aged 40 – 74, Norfolk resident or registered with a Norfolk GP, do not already have cardio vascular disease, diabetes or are taking statins (you may know this from their prescriptions).
Please don't ask your GP for medicines which can be bought at the pharmacy
This campaign is focussing on encouraging the public to purchase medications for short term ailments over the counter from a pharmacy or from their local supermarket. This will enable NHS resources to be prioritised for those who need it the most.
